Four Miyako nights provide three full days: Yonaha Maehama and the south coast, Irabu/Shimoji, and one free day. Another island group does not belong inside these five travel days.
JPY bands are rough planning estimates per person per day for two adults sharing a room, including food and local travel. Price flights, ferries, guided marine outings and major admissions separately; holiday and resort prices can differ substantially.

I would use one hotel for all four nights and separate the bridge outing from the swimming day. A coastal hotel is worthwhile only when its actual beach access matches the plan.

Choose the airport before the rental car
Miyako Airport and Miyako Shimojishima Airport are different airports. Check the full name on the flight and car booking, including pickup, opening hours and return. Road links allow Irabu and Shimoji to form part of one stay; changing airports still requires a planned transfer and does not make collection locations interchangeable.
Divide three days by coastal area
Use one day for Yonaha Maehama and a suitable stretch of the south coast. Drive to Irabu and Shimoji on another, selecting only a few stops. Bridges carry traffic: stop only at permitted parking places rather than pulling over on the carriageway for a photograph. Sunayama suits a separate short coastal visit when access is open. Keep the third full day for a favourite beach or to rearrange an outing after unsuitable conditions.
Choose a room around everyday needs
Compare evening meals, shopping, parking and the real approach to the water. A remote resort does not remove daily driving if you want a different dinner each night; a town base does not automatically offer a walkable swimming spot. Budget boat outings separately and book only with clear weather and cancellation terms. A free day remains useful without adding another activity.
